Enrollment Procedures
The child and family members may participate in any of the activities taking place throughout the classroom and are encouraged to explore the classroom and ask questions of the Director and staff.
Your Initial Visit
Families are encouraged to bring their children in prior to enrollment for an “introduction day”.
This time allows children and family members to become more familiar with the staff, other
children, and the daily classroom schedule.
Join the Waiting List
If there is not available space for a child or family in a classroom, a waiting list is formed.
Families on the waiting list are notified if space becomes available based on the order in which
they were put on the list, as well as their identified scheduling needs.

The following forms are included in the enrollment packet:
- Emergency Contact Form/Parent Consent Form
- Agreement Form
- Child Health Assessment
- All About Me Form
- Food Program Packet
- Keystone Stars Documents
Notes: The initial health form MUST be signed and dated by a doctor and returned within 30 days’ enrollment.
